REGISTER AS A NEW PATIENT?
You will need to attend the surgery and complete a registration form. If you can bring your medical card with you it would very helpful. When you register you will be asked to have a routine health check-up with one of our practice nurses. Whilst you will be registered with one particular doctor you can consult with any of the partners.
CHANGE MY PERSONAL?
Please advise one of our administrators if you change your name, address (including post code) or telephone contact numbers.
